Been about 75/25 for me, 75% empty, 25% troll present. Trolls do respawn at least in the outside, not sure about caves. The game DOES seem to simulate the "local troll" that came from the cave, as I've been in a cave that was empty with a troll nearby outside, when the troll outside disappears, I've gone in the cave and he was in there.

Here's the thing, I'm trying to lvl up my troll armour so have been hunting trolls. I've been in to roughly 10 or 11 troll caves and only found 2 with trolls. The rest have been empty. I don't recall such a high percentage of empty troll caves. I've tried going in day and night.
